There are several ways to contact CNS Medical Group, APMC: New patients are encouraged to use the email contact form on this website.

After completing the online contact form, below, you may also call the office at 925-784-4000 to leave a message with your email address and a new patient packet will be emailed to you shortly. Generally, the online contact form is the most rapid way of receiving information about the practice and to set up an new patient appointment. However, if you do not receive an email response in a timely manner, please call the office main number and leave a voicemail message. 

Established patients can call, send an email, or simply fill out the form on this page to send a routine (non-emergency) message.
